NP​/PA - Inpatient Hematology & Oncology

Christiana Care is seeking an experienced Nurse Practitioner or Physician Assistant to join our Inpatient Hematology and Oncology Team. This team of Advanced Practice Clinicians (APC) oversees admitted hematology and oncology patients including the Bone Marrow Transplant Unit at the Christiana Care Newark Campus. In this role, the APC will provide direct patient care while supporting admissions, new patient consults and cross‑coverage of inpatients on the Bone Marrow transplant and 6E units (Hematology/Oncology Floor).

This position is Monday-Friday, no call, no holidays, no weekends.

• Provide clinical work up and management to patients in the Hematology and Medical Oncology Inpatient practice which includes both oncology patients and benign hematology patients that are admitted to the oncology service or admitted patients that have oncology consult requested.
• Utilizes nurse navigation team to facilitate appropriate follow up with disease specific oncology team for any new diagnosis of malignancy and any admitted patient before discharge from the hospital.
• Performs and records initial history and physical assessment in response to consults, routine and acute clinical assessments, documentation of findings, and initiation of appropriate plan of care in collaboration with physicians and other members of medical stall.
• Adheres to all regulatory and organizational nursing policies and procedures to deliver excellent patient care.
• Works collaboratively with the clinical team, social workers, pharmacists, family members and others to ensure safe and seamless transitions of care.
• Visits newly admitted patients in a timely fashion per department policy.
• Individualizes medical care plan and transition of care to outpatient setting based on patient assessment.
• Provides appropriate patient education and counseling to patient and families in ways understandable to them.

• Certification as an Acute Care Nurse Practitioner (AGACNP or ACNP); or Current Board Certification as a Physician Assistant through National Commission on Certification of Physician Assistants (NCCPA).
• License or license eligible in the state of Delaware including controlled substance registration (CSR) eligible are required.
• BLS and ACLS, required
• Minimum 2 years of experience as a Nurse Practitioner or Physician Assistant, Oncology preferred
