- Hospital Care at Home Program is looking for a FT Registered Nurse (RN) to provide care to medical-surgical patients in the comfort of their homes. Our Hospital Care at Home Program enables patients to receive hospital-level care from the comfort of their homes rather than in ahospital. Our team of experts will treat and monitor patient health with the highest level of care.
Our Hospital Care at Home services include:
- A care team comprises physicians, nurse practitioners, physician assistants, paramedics, and registered nurses.
- Laboratory, infusion therapy, wound care, imaging, PT, OT, and EKG services.
- Advanced practice provider in-home visit.
- Virtual specialty provider consults.
- Medication delivery services.
While patients may not be physically present in the hospital, they remain continuously connected to their healthcare providers. Hospitalized at Home patients can connect with...
- (1) In-person Home Visits to Patient Hospitalized at Home (2 per week)
- (2) Virtual Visits
- (3) Monitoring through our Telehealth Platform.

Education & Experience:
- RN License in Delaware or a Compact State
- BSN preferredor committed to acquiring BSN within 3 years of the hire date.
- A minimum of three years of med/surg experience as an RN is required. Critical Care experience is a plus.
- BLS required.
- Active Drivers License and
PHYSICAL DEMANDS:
Ability to stand, walk, sit, bend, lift, push, pull, carry, reach, twist, handle, stoop, bend, and climb as per practice area-based Physical Demands Checklist.
WORKING CONDITIONS:
Frequent exposure to biological elements with occasional exposure to chemical and environmental elements as per practice area-based Physical Demands Checklist.

Hourly Pay Range: $42.11 - $67.37This pay rate/range represents Christiana Cares good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
