Licensed Clinical Social Worker (LCSW) – Saint James Health

Saint James Health Community Health Center (SJH) offers comprehensive, community-based preventive primary care and wellness services for New Jersey residents at three locations in Newark, NJ. This role focuses on delivering social work interventions within a multidisciplinary team.

• Provides social services and assistance to improve physical, social, and psychological functioning of patients and families.
• Identifies psychosocial status, economic and physical needs; assesses and reassesses patients’ support systems, community resources, and other factors to create an appropriate care plan.
• Provides psychosocial assessments, interventions, and care plans to patients, assisting them and families to cope with illness, diagnosis, and life situations.
• Identifies emotional or behavioral issues and diagnoses disorders using ICD-10 codes, screening tools, interviews, records, and reference materials.
• Counsel individuals and groups (if applicable) to modify behavior and improve personal, social, and vocational adjustment.
• Develops and implements individual treatment plans specifying type, frequency, and duration of therapy.
• Advocates for all community populations, respecting cultural and ethnic diversity and equity.
• Coordinates services to Health Center, community agencies, and other professionals; maintains working knowledge of and liaison with community agencies.
• Maintains continued professional development and keeps current with social work and health‑care developments.
• Collaborates with the health‑care team, involving patients/families in plan development and implementation.
• Facilitates patient care across a multidisciplinary continuum, interacting with clinic providers and other health‑care providers to manage patient movement and care arrangement.
• Serves as a patient advocate, maintaining knowledge of relevant medical/legal issues (e.g., advance directives, child and elder abuse).
• Uses the EMR system to maintain records and statistics per department policies.
• Attends and participates in staff committees, departmental clinical collaboratives, and administrative meetings.
• Contributes to the efficiency and effectiveness of the Health Center’s service by offering suggestions and acting as an active health‑care team member.
• Works collaboratively with Senior Management to resolve departmental and organizational challenges efficiently and with minimal disruption.
• Strives for excellence in daily operations of the department.
• Completes mandatory in‑services and attends required meetings.
• Adheres to Corporate Compliance policies.
• Ensures project boards () are clear, updated, and accessible to stakeholders, maintaining transparency of productivity.
• Performs other related duties as assigned.

• Master’s Degree in Social Work (required).
• Bachelor’s Degree in a related field.
• Three to five years as a social worker in a community health‑care environment, medical center, or similar setting.
• Active New Jersey license for Clinical Social Worker.
• Strong communication, clinical assessment, and case‑management skills.
• Ability to work collaboratively in a multidisciplinary team.
• Bilingual in Spanish or Portuguese preferred.
