What We’re Looking For
The Support Coordinator (DDD Program) plays a key role in helping individuals with developmental disabilities access and navigate services, resources, and support in the community. This position is responsible for assessing clients' needs, developing individualized service plans, coordinating services, and ensuring that clients receive appropriate care to enhance their quality of life. The Support Coordinator works in collaboration with clients, families, service providers, and community organizations to ensure that all needs are met effectively and efficiently.
What you'll do:
- Client Assessment and Service Planning: Conduct comprehensive assessments to understand the needs, goals, and preferences of individuals with developmental disabilities. Develop and update individualized service plans (ISPs) to ensure appropriate services are in place.
- Coordination of Services: Collaborate with service providers to ensure timely delivery of services, including but not limited to healthcare, vocational, residential, and educational support.
- Monitoring and Follow-Up: Regularly monitor clients' progress towards goals and the quality of services they are receiving. Make necessary adjustments to service plans based on changes in client needs.
- Advocacy: Advocate for clients to ensure they have access to all services and resources available through the DDD program, as well as other community-based resources.
- Documentation and Reporting: Maintain accurate and up-to-date records of client assessments, service plans, progress notes, and any other documentation required by the DDD program. Ensure compliance with state and federal regulations.
- Collaboration and Communication: Work closely with families, caregivers, healthcare professionals, and other stakeholders to coordinate care and ensure all aspects of the client’s needs are met.

Education:
Provide information and education to clients and their families about available services, rights, and community resources. - Crisis Management: Assist clients and families in addressing urgent needs and crises, providing timely interventions and coordination of necessary services.
- Compliance: Ensure all services and actions comply with federal, state, and local laws, including the policies and procedures of the DDD program.
The Ideal Candidate:
- Bachelor's degree in Social Work, Psychology, Special Education, or related field (required).
- Minimum of 2 years of experience working with individuals with developmental disabilities, including experience in case management or service coordination.
