Licensed Practical Nurse (LPN)

Schedule:

Weekends, 8 hr shifts

Duties and Responsibilities:

Provide skilled nursing care to patients in their homes

Conduct comprehensive assessments of patients' health status

Develop and implement individualized care plans

Administer medications and treatments as prescribed by physicians

Monitor and evaluate patients' progress and adjust care plans accordingly

Educate patients and their families on managing their conditions and promoting wellness

Collaborate with other healthcare professionals to coordinate patient care

Document patient care activities accurately and timely

Experience:

Preferred minimum of 1 year of experience as an LPN in a home health setting

Experience with tube feeding and managing enteral nutrition

Knowledge of sterile processing techniques and infection control protocols

Experience working with children in a healthcare setting preferred
