Form Health is a virtual obesity medicine clinic delivering multidisciplinary evidence‑based obesity treatment through telemedicine. The role is full‑time, remote, and involves providing longitudinal telehealth care for patients with obesity, collaborating with physicians, advanced practice professionals and registered dietitians.
ResponsibilitiesProvide evidence‑based lifestyle counseling and pharmacotherapy for obesity patients; conduct telemedicine visits with effective web‑side manner; work closely with care team members to deliver team‑based care; document patient encounters and coordinate care plans.
Qualifications- Have a Nurse Practitioner degree with experience practicing obesity medicine, including evidence‑based lifestyle counseling and pharmacotherapy.
- Licensed to practice and prescribe legally; preference for credentialing with insurance payers in New Jersey.
- At least 2 years of experience in outpatient or obesity medicine, endocrinology, bariatrics, or a primary care or internal medicine setting.
- Excellent interpersonal skills for patient and team interactions.
- Committed to the highest quality of medical care and an evidence‑based approach to obesity management.
- Prior experience conducting telemedicine visits and possessing a great web‑side manner.
- Bilingual candidates with medical fluency in Spanish are highly preferred.

SalaryBase Annual Salary: $110,000—$125,000 USD.
