, we're committed to providing compassionate, high-quality healthcare services in the comfort of our patients' homes. We're seeking a dedicated and motivated Physical Therapist (PT) to join our growing team and help patients improve their mobility, independence, and overall quality of life.

• Deliver physical therapy services in accordance with physician's plan of care and state PT practice guidelines
• Assess and evaluate patients' therapeutic, rehabilitative, and functional status
• Develop and direct individualized therapy programs focused on rehabilitation and recovery
• Evaluate home environments and recommend safety/therapy modifications
• Train patients and caregivers in therapeutic appliances and prosthetic device use
• Provide instruction and support to families/caregivers and collaborate during case conferences
• Supervise and guide Physical Therapy Assistants (PTAs) per policy and regulations
• Monitor and report patients' responses to treatment, adjusting plans as needed
• Prepare clinical and progress summaries, participate in discharge planning, and coordinate with interdisciplinary teams

• Active Physical Therapy license in Indiana and/or Ohio
• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program
• CPR certification and recent TB test required
• Valid driver's license, reliable transportation, and auto insurance
• 2+ years of physical therapy or rehab experience preferred (community/home health experience ideal)
• Strong communication, organizational, and leadership skills
• Ability to build rapport with patients and handle physical workload

• Salary
  : $88,500 - $120,000/year (Mid-Range: $104,000)
