Physical Therapist (AL/IL & Home Care) (9483)

Schedule: Full-Time – Monday to Friday (40 hours/week), flexible scheduling.

Salary: $80,000 – $110,000 per year.

Travel Required: Yes – mileage reimbursed at $0.48 per mile.

• Support and treat patients in assisted living (AL) and independent living (IL) facilities, and perform in-home visits.
• Deliver physical therapy services per physician’s care plan and state regulations.
• Assess patient mobility, strength, and rehabilitation potential.
• Design and implement personalized therapy programs.
• Educate patients and caregivers on therapeutic techniques and equipment.
• Collaborate with interdisciplinary teams to ensure holistic care.
• Supervise PTAs and home health aides as needed.
• Document progress and participate in discharge planning.

• Active Ohio PT license.
• Degree in Physical Therapy (Bachelor’s, Master’s, or Doctorate).
• CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.
• Recent TB test.

• 2+ years of PT experience.
• Home health or community-based care experience.
• Strong communication and organizational skills.
• Ability to build rapport and lead with empathy.
