Position Summary

Under supervision, and according to state and federal law covering the practice of pharmacy, establishes policies and procedures; compounds and dispenses medications and other pharmaceuticals supplies. Provides a pharmaceutical service and education to a patient population that includes ages ranging from newborn through geriatric. Serves as a consultant to other Hospital Departments, such as Cardiac Rehabilitation.

Responsibilities

  • Monitor patient medications to determine possible medication therapy problems
  • Serve as Interventional Pharmacist competently per Pharmacy Practice Guidelines.
  • Supervise and/or prepare intravenous admixture solutions according to physician orders.
  • Check work of technical staff for accuracy
  • Understand emergency drugs and thrombolytic therapy (including continuous infusion, normal dilution, dose and monitoring during infusion).
  • Respond to Code Blues, Traumas, STEMI's, and Rapid Sequence Intubations in the ED and throughout the hospital. Prepare prescribed medications, including compounded formulations and IV's
  • Perform Medication Reconciliation on patients admitted to the hospital and other ED patients at physician request. Write Pharmacist Consults to hospitalists that identify medication issues that may be problematic for the patients. Educate patients on correct medication use
  • Evaluate Cultures and sensitivities for discharged Emergency Department patients. Make recommendations to physicians on any treatment changes needed. Notify patients of culture results and call any required prescriptions into area pharmacies
  • Provide drug information to physicians, nurses, and patients.
  • Supervise ED Certified Pharmacy Technicians by utilizing good team-building skills, in order to unite ED Pharmacy staff around common goals and generating greater productivity.

Requirements

  • Pharm D or Bachelor's degree in pharmacy.
  • Licensure as a Pharmacist in the State of Ohio.
  • Epic experience beneficial
  • Analytic ability to determine drug appropriateness and evaluate drug instructions.
  • Ability to provide care for a neo‑natal, pediatric, adolescent, adult and geriatric patient population.
  • PALS and ACLS Certification preferred (training will be provided)
  • LMH is accredited by DNV and TJC, and as such, may require specific annual education related to specialty certifications and standards.
