Role Description

The LSW/LISW or LPC/LPCC in TBS & Mental Health Groups will provide individual and group therapeutic services in a nursing home setting, focusing on residents’ emotional, behavioral, and psychosocial needs. This full-time, on-site role is based in Newark, OH and involves conducting assessments, developing treatment plans, facilitating therapeutic groups, and documenting services in accordance with regulatory and organizational standards. The clinician will collaborate closely with nursing home staff, medical providers, and families to coordinate care and support resident-centered treatment goals.

Day-to-day responsibilities include ongoing clinical evaluation, crisis intervention as needed, participation in care team meetings, and contributing to a supportive, recovery-oriented environment. The role also includes maintaining ethical practice, adhering to evidence-based modalities, and engaging in regular supervision and professional development.

Qualifications
  • Active Ohio licensure as LSW/LISW or LPC/LPCC in good standing, with eligibility to provide services in a nursing home setting.
  • Demonstrated skills in individual therapy, group therapy facilitation, and treatment planning for adults and older adults.
  • Experience with behavioral health conditions, dual diagnosis treatment, and recovery-oriented care in long-term or residential settings.
  • Strong assessment, clinical documentation, and case management skills, including compliance with regulatory and payer requirements.
  • Ability to collaborate effectively with interdisciplinary teams, including nursing, medical providers, and family supports.
  • Excellent communication, boundary-setting, and de-escalation skills, with a compassionate, trauma-informed approach.
  • Proficiency in using electronic health records (EHR) and basic computer applications for clinical documentation.
  • Master’s degree in Social Work, Counseling, or a related behavioral health field preferred; relevant experience in nursing home or geriatric mental health strongly beneficial.
  • Commitment to evidence-based practice, cultural humility, ethical standards, and ongoing professional development.
