Base pay range$44,600.00/yr - $61,400.00/yr
Compensation detailsCenter Well Home Health provides a pay range based on skills and experience. Your actual pay will be discussed with your recruiter.
Responsibilities- Provide professional nursing services under the direction of an RN in compliance with the state Nurse Practice Act, licensure/certification requirements, healthcare standards, laws and Center Well policies.
- Provide skilled nursing interventions for illness, rehabilitation, and preventive care. Deliver holistic, problem-specific therapeutic interventions, teaching and training in the plan of care.
- Apply knowledge and skills per accepted clinical standards to facilitate problem resolution and meet patient goals.
- Consult with the RN Supervisor regarding changes in the Plan of Treatment. Accept verbal orders from physicians as permitted by state law and communicate them to the RN Supervisor.
- Implement the nursing plan of care using a systematic, individualized, goal/outcome-driven approach.
- Maintain timely contact with patients, physicians, clinical managers and other team members regarding patient needs and status changes. Participate in care coordination and discharge planning as appropriate.
- Maintain confidentiality and professional conduct regarding patient information.
- Communicate to ensure availability of supplies, materials and equipment required to implement the plan of care safely and effectively.
- Prepare, submit and maintain documentation as required by the Company and/or facility. Visit/shift notes documented on day services as rendered.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid driver’s license, auto insurance and reliable transportation.
- Current CPR certification.
- Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
- Pay range per visit/unit: $28.00 - $40.00
- Base pay: $44,600 - $61,400 per year
- Scheduled weekly hours: 40
