The Role Responsibilities
- Coordinating the day-to-day operation of the reablement service, ensuring the smooth delivery of reablement interventions to clients.
- Leading and motivating a team of dedicated support workers to provide high-quality care and support to individuals.
- Working collaboratively with other professionals, such as social workers and healthcare providers, to ensure holistic and integrated care for clients.
- Conducting assessments and reviews of client needs, identifying goals and outcomes for reablement.
- Monitoring and evaluating progress, providing guidance and support to clients to achieve their desired outcomes.
- Ensuring accurate and timely documentation, including care plans, progress notes, and statistical information.
- Participating in team meetings and contributing to the overall development and improvement of the reablement service.
- Experience working within a reablement or rehabilitation setting, with a sound understanding of the principles and practices of reablement.
- Strong leadership and people management skills, with the ability to motivate and inspire a team.
- Excellent communication and interpersonal skills, with the ability to build positive relationships with clients, colleagues, and external stakeholders.
- Sound assessment and care planning skills, with the ability to identify individual needs and set achievable goals.
- Ability to work collaboratively within a multi-disciplinary environment, demonstrating effective partnership working.
- Organised and proactive, with the ability to prioritise tasks and manage competing demands.
- Knowledge of relevant legislation, regulations, and best practices related to reablement and social care.
