Schedule:

Every Fri. and Sat. plus one weekday
Coverage area:
Newburyport and surrounding areas

Responsibilities
  • Promote Elara Caring’s philosophy, mission statement and administrative policies to ensure quality of care.
  • Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
  • Assess assigned visits of hospice patients to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation in the clinical record and admission paperwork.
  • Participate in the implementation of the plan of care for an assigned case load to ensure quality, continuity of care, and achievement of patient outcomes.
  • Make the initial evaluation and re‑evaluate the patients’ nursing needs during each visit.
  • Communicate significant findings, problems, and changes in condition or environment to the Patient Care Manager, the physician, and/or other personnel involved with patient care within 24 hours.
  • Report unsafe conditions to the appropriate Clinical Supervisor and physician in a timely manner.
  • Identify deviations from normal nursing and normal patient condition status. Revise the plan of care and initiate appropriate actions based on the deviation.
  • Perform other duties/projects as assigned.
Qualifications
  • Graduate of an accredited nursing program.
  • Current, unrestricted state RN licensee.
  • 1+ year experience in a clinical care setting as a nurse.
  • Able and willing to travel within branch/office coverage area.
  • Must have a dependable vehicle, valid driver’s license and current auto insurance in accordance with state laws.
  • Must be able and willing to travel 50%.
  • Associates Degree or Bachelors Degree in Nursing is preferred.
  • Previous Home Health or Hospice Experience is preferred.
  • Previous experience with Homecare Home Base  is preferred.

Reporting to:
Clinical Team Manager or Branch Director.
