INTERNAL MPFT APPLICATIONS ONLY Senior Community Nurse
These posts are available to internal MPFT Employees only.
What we ask from you?- Working in a multi-disciplinary team alongside Clinical and Operational Leadership to ensure high quality care delivery to service users.
- Provide specialist assessment and care planning in conjunction with service users and carers.
- Evaluate treatment plans and undertake discharge planning and promote self care.
- Be responsible for an agreed caseload.
- Co-ordinate the work of the Team.
- Support development of skills and competencies in others.
- Nursing Registration with the NMC
- Degree in nursing or equivalent
- Mentorship/Assessor Qualification
- Essential experience
- Experience in leadership
- MDT working
- Caseload Management
- Clinical skills relevant to community nursing - fully completed skills passport
