Tyneside and Northumberland Mind, a local mental health charity, are hiring a
HISU Project Lead who will oversee our HISU Pilot Project. The role will include supporting adults who frequently attend the emergency departments, 111 option 2 and local PCNs within Newcastle and Gateshead.
The HISU Project Lead will coordinate all activity from Senior Link Workers, Link Workers into clinical settings, attend and arrange referrals from primary and secondary care settings, and be responsible for ensuring data is collected and reported in a timely and appropriate fashion for evaluation of this pilot.
Based at:Hybrid – across Newcastle and Gateshead
Contract length:15-months with potential for extension
