this is an exciting opportunity to join our transformation journey as a service planning manager working within our better together transformation portfolio. We are seeking two highly skilled individuals to play a key role in leading the service planning requirements to deliver a new transformational model of care for powys. We are specifically looking for these roles to support phase 1 of our better together journey, with a focus on adult physical and mental health community services.

A background in service planning in these areas would therefore be desirable. This work be key to the delivery of community by design, 'a healthier wales', and our health & care strategy through the development of a clinical services plan for powys. There will be a need to cross‑cover and to provide service planning expertise and support to other high‑priority service areas, such as planned care, as required and as the transformation portfolio develops.

duties of the job

the post holder will require a level of freedom to act, to enable agile decision making and actions; they will be expected to interpret national policy and guidance for health and social care. This role will operate in a complex environment, working across health, social care and multi‑agency organisations to ensure the implementation of national and local policy across directorates. The post holder will work closely with operational and corporate services and will be responsible for analysis of detailed, complex datasets and be able to undertake research in relation to national and local evidence‑based to make judgements on how to effectively deliver the new model of care at both a strategic and operational level.

the role will involve working with clinicians, managers, staff, professionals, partners, and other key stakeholders across health, social care, and multi‑agencies to deliver a new model of care.

key responsibilities of the role will include:

undertake mapping of existing services and development of a detailed, robust case for change required as part of options and business case development.

leading on the development of new models of care, service plans/specifications and pathways in conjunction with health and care professionals, based on an agreed planning framework, national guidance and legislation, powys health and care strategy and area plan and local population needs and intelligence.

significant amount of engagement and communication messaging.

work with the executive leads, senior responsible officers, transformation programme managers, operational and clinical leads and wider team members to provide leadership and expertise in technical planning to deliver transformational change in powys.
