Dorset Health Care University NHS Foundation Trust is seeking an experienced practitioner to work across Primary Care and the Access Wellbeing Service within the Bournemouth and Christchurch Locality. You will screen, triage and advise on referrals into the Access Wellbeing model and help progress patient pathways.
You will engage with virtual MDT meetings, collaborate with Mental Health Teams and primary care colleagues to route patients to the most appropriate service.
