Job overview

We are seeking a skilled, motivated and compassionate Community Practitioner to join the Newcastle & Gateshead Step Up Hub. This is an exciting opportunity to work within a dynamic multidisciplinary team, supporting individuals to remain safely within their community and avoid unnecessary hospital admission. Applicants must be registered nurses with a current professional registration.

This post offers a rewarding mix of community and hospital in-reach responsibilities, with a focus on delivering high-quality, rehabilitation-oriented care.

In this role, you will provide care coordination and nursing support to service users with complex mental health needs across both community and inpatient settings. You’ll play a crucial part in ensuring smooth transitions between hospital and community care, attending Mental Health Act tribunals and working closely with multidisciplinary teams.

You will need to be clinically experienced, solution focussed and competent in working with individuals, their families, and carers. You should have a collaborative and enthusiastic approach as well as the passion and commitment to develop and promote recovery-based interventions for adults with mental health related needs.

We want you to be committed to ensuring that only the highest standards of care are delivered to our service users, their families and carers. In return one of our top priorities is looking after our staff so that we have a happy and healthy workforce.
