Rethink Mental Illness is seeking a Care Navigator based at HMP Durham, with an agile role and full UK driving licence. You will work in integrated mental health teams, providing 1:1 and group assessments to support people leaving custody and rejoining the community.
Using coaching techniques, you’ll help maintain wellbeing, map care needs, and connect individuals with health and community services to reduce ill health and reoffending. This role requires flexibility and professional boundaries.
