Overview
The Registered Nurse (RN) role models clinical excellence and the core competencies of the practice standards of the College of Nurses of Ontario (CNO), as well as the policies and procedures of Southlake Regional Health Centre (Southlake). The RN works collaboratively as a member of the health care team and provides direct patient care to patients based on the three-factor framework outlined in the CNO standard, RN and RPN Practice:
The Client, Nurse and the Environment. These three factors impact decision making related to care-provider assignment and the need for consultation and collaboration among care providers. All nurses commit to lifelong learning.
The successful candidate will be motivated, self-directed and a key member of a dynamic interdisciplinary team with defined roles focused on quality patient care and patient flow. The successful candidate will be responsible for providing excellent care to a diverse acute medical patient population in a fast-paced setting.
The Heart Function Program Nurse will work with the heart function team to provide optimal treatment and care to congestive heart failure patients. This position includes clinical assessment, coordination of the patient plan of care, family and patient education, and maintaining knowledge to reflect current standards of care within the congestive heart failure population.
Responsibilities- Provide direct patient care to patients based on the three-factor framework (The Client, Nurse and the Environment) as outlined by the CNO standard RN and RPN Practice.
- Collaborate as a member of the health care team and engage in consultation and collaboration with care providers as needed.
- Support quality patient care and patient flow within a fast-paced, acute medical setting.
- Work with the heart function team to provide optimal treatment and care to congestive heart failure patients, including clinical assessment, coordination of the patient plan of care, and patient and family education.
- Motivated, self-directed, and a key member of a dynamic interdisciplinary team with defined roles focused on quality patient care and patient flow.
- Ability to provide excellent care to a diverse acute medical patient population in a fast-paced setting.
- Knowledge and competence in current standards of care within congestive heart failure population.
