FOOT CARE PRACTITIONER – RPN

The AFPN RPN plays a pivotal role in enriching the lives of residents by supporting the LTC Home in the administration of treatments and assessments. The Foot Care Practitioner will provide routine foot care nursing services to residents and clients within the portfolio.
Participates in all multidisciplinary meetings to ensure excellence in resident care and uphold the quality of nursing practice in accordance with the College of Nurses of Ontario Standards, Long-Term Care Standards, and Southbridge Care Homes Standards.
Responsibilities- Ensure resident care is provided with established nursing standards and principles, physician’s orders, and administrative policies, and observe, report symptoms, and conditions of residents, and initiate remedial health measures where appropriate.
- Assess residents and contribute to resident care plans, monitor residents’ health, note changes in condition, and assess need for referral to doctors.
- Perform foot‑care procedures for which recognized training and current competency can be demonstrated. Observe and document client conditions and changes following proper standards and in accordance with professional standards.
- Advanced and Diabetic Foot Care Certification is required.
- Registration or eligibility for registration with the Registered Practical Nurses Association of Ontario.
- Access to reliable transportation and a G class license.
- Willingness to travel between work sites if required.
- Comprehensive knowledge of nursing and health‑care practices, including infection control practices.
- Effective human relation skills and demonstrated commitment to working with seniors.
- Effective communication skills and ability to work independently and collaboratively.
- Negative vulnerable sector check completed within 6 months of start date.
- Completion of a 2‑step TB skin test within the last 12 months of start date.
