RPN Care Coordinator at Southbridge

Join Southbridge Care Homes as a Registered Practical Nurse focused on coordinating care for residents. You'll deliver high-quality nursing services while working day and evening shifts.

This position directly reports to the Director of Care and involves assessing residents, documenting vital signs, and implementing physician orders. You will also organize care functions and lead a team of caregivers, ensuring compliance with nursing standards.

Key Responsibilities:

• Observe and document residents' vital signs and health conditions

• Create and evaluate personalized care plans for residents

• Direct restorative and palliative care programs

• Supervise nursing staff and support new employee training

• Review staff performance and address issues as needed

Requirements:

• Certificate of competence from the College of Nurses of Ontario

• Must be registered or eligible with the Registered Practical Nurses Association

• First Aid and BLS Certificate required

• In-depth knowledge of nursing and health care practices

• Excellent communication and collaborative working skills
