We are searching for dedicated Registered Nurses (RN) and Practical Nurses (RPN) to join our team. This role focuses on community-based care, allowing you to build strong relationships with your clients. You will conduct comprehensive assessments, develop personalized care plans, and lead care transitions while collaborating closely with team members.
Key Responsibilities:
• Complete detailed client assessments and formulate care plans
• Act as primary provider for assigned clients
• Collaborate with other health professionals for comprehensive care
Requirements:
• Current registration with the College of Nurses of Ontario
• Valid CPR certificate and driver's license
• Physical ability to lift up to 55 pounds
• Availability for weekdays, evenings, and every other weekend
