As a Discharge Nurse, you will coordinate discharge processes across various inpatient programs, reporting to the Patient Access & Flow Department. Your expertise ensures a patient-centered approach that optimizes flow, improves outcomes, and aligns with hospital goals. You’ll regularly engage with patients and families, ensuring seamless transitions between care environments.
Key Responsibilities:
• Facilitate implementation of patient-centered discharge plans
• Collaborate with clinical teams to adhere to discharge policies
• Provide education for patients and families on follow-up care
• Document discharge activities in Meditech and other systems
• Participate in quality improvement initiatives for discharge processes
Requirements:
• Registered Nurse with Ontario College of Nurses
• Minimum five years of Emergency nursing experience
• Two years in discharge planning preferred
• Strong communication and interpersonal skills
• Baccalaureate Degree in Nursing preferred
