Nurse Practitioner

Position: Nurse Practitioner at Southlake Health
Enhance patient care as a Nurse Practitioner with Southlake Health in Newmarket, ON. This part-time role encompasses advanced clinical practice, patient education, and collaboration in a supportive hybrid work environment.

The Nurse Practitioner will engage in direct and indirect patient care within internal medicine, promoting health and chronic disease management. Responsibilities include evaluating patient health statuses and implementing evidence-based practices, while ensuring adherence to medication regimens. This position offers the opportunity to provide leadership and high-quality care in collaboration with healthcare teams.

Key Responsibilities:

• Conduct comprehensive health evaluations for chronic illnesses

• Track and evaluate patient symptoms proactively

• Educate patients on medication compliance and side effects

• Diagnose new health conditions within primary care scope

• Coordinate referrals for specialist care and support

Requirements:

• Bachelor of Science in Nursing;
Master's in Nursing/Nurse Practitioner preferred

• Current registration with the College of Nurses of Ontario

• Minimum 3 years of related clinical experience

• Demonstrated collaborative practice with multidisciplinary teams

• Ability to prioritize tasks and work independently
