Key Responsibilities:
• Conduct assessments for resident health and wellness
• Create and revise personalized care plans
• Collaborate with multi-disciplinary teams for care delivery
• Manage medication distribution and documentation
• Address health and safety concerns as needed
Requirements:
• RPN license, registered with the College of Nurses of Ontario
• High school diploma required
• Emergency First Aid, CPR, AED certifications necessary
• Proficient in written and verbal communication
• Prior nursing experience in a care environment preferred
