Key Responsibilities:
• Develop and evaluate care plans tailored to clients' unique needs
• Lead care for assigned client caseloads and coordinate with teams
• Assist clients in navigating community support services
Requirements:
• Current registration with the Ontario College of Nurses
• Certification from CNA is advantageous
• Required CPR certification and a valid driver's license
• Updated immunizations and background checks mandatory
• Must be able to perform physically demanding tasks
