Key Responsibilities:

• Monitor and evaluate symptoms to prevent hospitalizations

• Conduct comprehensive health evaluations for chronic patients

• Facilitate smooth transitions between care levels

• Maintain accurate documentation in EMR system

Requirements:

• Bachelor’s and Master’s in Nursing required

• Active CNO registration as Nurse Practitioner

• Minimum 5 years of relevant experience
