The CRT role models clinical excellence and the core competencies outlined by the College of Nurses of Ontario (CNO), while adhering to the policies, procedures, and values of Southlake Health.

The CRT member works collaboratively as a valued member of the interdisciplinary health care team, providing safe, high-quality patient care guided by the CNO's Scope of Practice, Practice Standard.

As a member of Southlake's Clinical Resource Team (CRT), you will join a dynamic, highly skilled team dedicated to quality patient care, patient flow, and organizational responsiveness.

CRT staff play a critical role in supporting multiple inpatient programs and developing broad clinical expertise across diverse patient populations and care environments.

The CRT operates under a Graduated Practice Framework, where progression is achieved through demonstrated competence, successful completion of required competencies, and readiness for increasing levels of responsibility.

This competency-based approach supports patient safety while providing team members with a structured pathway for professional growth and development.

CRT staff provide patient care across a variety of inpatient clinical areas, including but not limited to Emergency, Medicine, Maternal Child, Surgery, Cardiac, Mental Health, Restorative Care, and other specialty areas based on individual skills, experience, education, and demonstrated competencies.

What you will be doing:
  • Provides patient and family-centered care within the RN scope of practice in accordance with the CNO Professional Standards, Code of Conduct, and evidence-informed practice.
  • Functions in accordance with established hospital policies and procedures, including emergency codes.
  • Ensures that practice is consistent with entry to practice competencies, standards of practice, guidelines and legislation.
  • Communicates effectively with patients, families and all members of the healthcare team.
  • Practices collaboratively with members of the healthcare team to provide care for patients/clients with complex health issues.
  • Provides consultation and/or clinical support to peers as needed.
  • Contributes to the assessment and documentation of each patient's health status on an ongoing basis.
  • Assists in the development and implementation of the Plan of Care and evaluates its ongoing effectiveness.
  • Assists in the orientation/preceptorship of new staff members and students to the clinical units.
  • Acts as a role model and champion to identify and integrate safe, best practices into daily activities to foster the delivery of safe and exemplary care.
  • Takes a leadership role in the coordination of patient care in assigned area.
  • Assumes charge nurse role when assigned.
  • Effectively utilizes human and material resources.
  • Participates in the development, implementation and evaluation of clinical pathways.
  • Provides consultation and/or clinical support to peers as needed.
  • Other duties as assigned by Manager/delegate.
What you bring:
  • RN Certificate of Registration from the College of Nurses of Ontario
  • Current BLS certification required
  • ACLS Preferred
  • CC1 or equivalent to be successfully completed during Orientation to CRT Level 3, in house course provided if required.
