Southbridge Care Homes is looking for a skilled Foot Care Practitioner-RPN to work at Eagle Terrace LTC. This on-call position demands an Advanced and Diabetic Foot Care Certification and the ability to communicate effectively with residents and staff. Practitioners will be actively involved in resident assessments, care plans, and providing routine foot care services.

Key Responsibilities:

• Ensure care aligns with nursing standards and protocols

• Monitor residents and assess health needs

• Document and observe changes in client conditions

• Facilitate assessments and referrals as needed

• Collaborate in multidisciplinary meetings for care excellence

Requirements:

• Advanced and Diabetic Foot Care Certification required

• Registration with the Registered Practical Nurses Association of Ontario
