The Role
Reporting to the Manager, Community Programs, the Nurse Coordinator plays a key role in the coordination, delivery, and ongoing management of Southlake Health's established Southlake@home programs. Working collaboratively with patients, families, caregivers, physicians, interprofessional hospital teams, Ontario Health @Home (OH@home), and contracted community service providers, the Nurse Coordinator identifies and assesses eligible patients and facilitates safe, timely transitions from hospital to home.

The Nurse Coordinator is responsible for facilitating referrals, assessing program eligibility, coordinating discharge planning, confirming appropriate community services and equipment, and monitoring patient progress in collaboration with contracted service providers. When a patient's clinical or functional needs change, the Nurse Coordinator works with the appropriate healthcare professionals to reassess care requirements, facilitate timely escalation when indicated, and coordinate transitions to alternative services, including OH@home, where ongoing support is required.
As an integral member of Community Programs, the Nurse Coordinator supports patient flow, reduces avoidable hospital days and Alternate Level of Care (ALC) pressures, and promotes safe, effective care in the home.
