Job Region: All Blue Door Locations, Hybrid

Schedule: Sunday to Thursday, 8:30-4:30. With some flexibility.

Equipment: Computer, telephone, Reliable Vehicle/Valid Driving License/Insurance.

Salary: $50,000-$55,000 annual salary

Blue Door is a not-for-profit registered charitable agency governed by a volunteer Board of Directors. We have been an integral part of our community providing essential emergency shelter services since 1982. Blue Door’s mission is to support people who are at risk of, or experiencing homelessness to attain and retain affordable housing.

Job Summary

The Housing Stability Coordinator is part of the Housing to Health project, a new collaborative project between Blue Door, Krasman Centre and LOFT Cross links Housing and Support Services. The Coordinator provides intensive case‑management supports to individuals facing severe struggles including homelessness, mental health challenges, physical health impairment, legal and justice issues, historic discrimination, trauma, victimization, addictions and stigma. The role involves creating a support network and linking consumers to community resources, ensuring complex‑need clients receive the necessary supports to reintegrate into the community, retain housing, and achieve their goals.

Responsibilities
  • Implement intake process for clients; conduct assessments, develop support and recovery plans, and identify priorities and goals in conjunction with professionals or intake staff.
  • Communicate client goals and plans to Housing to Health team members and shelter teams to ensure seamless support.
  • Promote personal empowerment through coaching, mentoring, and support, maintaining consistent communication, scheduling regular meetings, and tracking client outcomes.
  • Directly support daily living activities and develop ongoing plans for housing searches and wrap‑around services to promote retention.
  • Promote capacity building in life‑skill areas such as hygiene, medication management, cooking, cleaning, shopping, and budgeting.
  • Facilitate group workshops, programs, and individual home/community visits.
  • Establish and coordinate a support network of social, medical and other community links, maintaining linkages through non‑intrusive follow‑up, reminders, and occasional accompaniment to appointments.
  • Establish a crisis plan and provide 24‑hour crisis support as needed via existing service connections.
  • Build, maintain and strengthen working relationships with community service providers, expanding the existing list of supports.
  • Facilitate, provide leadership, and collaborate in internal case‑management meetings and meetings with other service providers.
  • Advocate for clients navigating social services and healthcare systems, ensuring accessibility and availability of services, mediating issues, teaching conflict resolution skills, and following up for resolution.
  • Consistently maintain, update, and monitor client records and provide additional supports as required.
  • Work closely with Peer Support Workers and Housing Locator.
  • Follow up with collaborating organizations and partnering agencies to ensure data for Housing to Health clients is properly recorded, maintained, and submitted to Blue Door Shelters.

  • Foster positive and appropriate relationships with clients, colleagues, community partners and volunteers.
  • Provide referrals and follow‑up as necessary.
  • Meet with manager regularly and participate in completion of annual performance evaluation.
