Summary:
York Region South Simcoe (YRSS) Supportive Housing Services provides support to individuals facing serious mental health challenges and homelessness, who might also experience substance use challenges. This program provides a range of supportive housing options across York Region and South Simcoe, either within LOFT owned houses/buildings or through head lease arrangements with private landlords. Each house or apartment, whether individual or shared, comes fully furnished and with support services attached.
Our goal is to support clients to maintain housing stability and tenure in the community, in a manner that fosters self-worth and dignity.
We offer a range of programming that meets the level of support needs or unique challenges of specific populations. These include small group living arrangements for those requiring minimal support, individual and shared accommodations, and staffed congregate settings for individuals needing daily support. Within that continuum, we also serve people with current involvement in the criminal justice system or those with several comorbid health issues.
Job Summary:
Through a team oriented professional approach, the position provides support to the client to increase their independence, dignity and self-respect, in a safe and secure environment by working within a team-based environment, you will support clients in achieving greater independence, dignity, and well-being within a safe and supportive housing setting.
What You Will Do:
- Client Support & Skill Building:
Build strong, respectful relationships with clients, providing coaching and hands-on support in daily living activities such as personal care, medication management, meal preparation, budgeting, and maintaining a healthy lifestyle. - Program & Household Operations:
Facilitate house meetings and group activities, support client engagement, assist with daily household operations, and ensure a safe, clean, and well-maintained living environment. - Community & Service Coordination:
Help clients navigate healthcare and social service systems, maintain community partnerships, and support or accompany clients to appointments as needed, including participation in 24 hour crisis support. - Conflict Resolution & Recovery Support:
Promote a psycho-social rehabilitation / recovery-oriented approach, including harm reduction, conflict resolution, and problem-solving to support positive shared living environments. - Administrative & Team
Collaboration:
Maintain accurate documentation, contribute to program planning and evaluation, support team communication, and assist with scheduling, training, and continuous improvement initiatives.
- A relevant degree/diploma in human services or a related field.
- At least 2+ years of experience working with the vulnerable populations.
- Knowledge of the challenges faced by clients including those experiencing mental health issues mental health, addictions, cognitive impairments, HIV/AIDS etc.
- Strong understanding of harm reduction and psychosocial rehabilitation principles.
- Strong crisis intervention, problem-solving, and conflict resolution abilities.
- Ability to build therapeutic relationships and collaborate with clients on goals and care plans.
