Job Summary

The Newmarket Integrated Neighbourhood Team is looking for a part‑time District Nurse within the INT, to work alongside an established team of one other District Nurse & several Community Nurses, OT’s and Physiotherapists with support staff. You will be at the front line of healthcare, delivering medical care to patients on our caseloads in their own homes, supported by an Integrated Community Team that includes Adult Social Services staff.

• Lead and manage the District Nursing Service in the locality, providing specialist clinical and supervisory leadership to junior staff and students.
• Manage a caseload of patients with multiple and complex needs, using evidence‑based practice to assess, plan, implement, and evaluate interventions, with the patient at the centre.
• Provide the highest standard of clinical care by using advanced skills and expert knowledge to holistically assess needs and provide clinical treatments.
• Lead service development of integrated services in partnership with peers and colleagues.
• Use new knowledge in innovative ways and take responsibility for developing and changing practice in complex and sometimes unpredictable environments.
• Support staff to feel confident and competent to move across organisational and professional boundaries.
• Monitor the quality of care provision and identify and promote areas for service development.

• Registered Nurse Level 1
• District Nurse Qualification
• Non‑medical prescribing qualification
• Preparation for Mentorship qualification
• Current NMC registration
• Evidence of continuing professional development
• Teaching, learning and assessing qualification

• District Nurse qualification at Masters Level
• Specific Leadership qualification
• Tissue viability or leg ulcer management training
• Specific management qualification
• V300 non‑medical prescribing qualification
• Teaching, learning and assessing, Level 7 or PGCE

• Experience of leading a DN team
• Management of complex patients
• Experience of managing complex cases and sharing knowledge with locality and community nurses
• Advanced clinical assessment skills, history taking and physical examination
• Experience of working autonomously
• Experience of working with people with long‑term conditions
• Awareness of local and national priorities in relation to patients with long‑term conditions, nursing and integration
• Ability to direct and co‑ordinate programmes of care working collaboratively and autonomously
