Gentiva is seeking a compassionate Hospice RN / RN Case Manager to lead patient care in home and community settings. You will perform comprehensive assessments, implement and document Plan of Care, and coordinate with physicians, LPNs, CNAs and social workers to ensure high-quality end-of-life care with empathy.
You will educate families, support emotional and spiritual needs, and participate in on-call activities while maintaining accurate records and complying with regulatory standards.
