The Home Health/Hospice Nurse I (RN) provides direct patient care in accordance with the Nurse Practice Act to assigned patients. The nurse collaborates with other disciplines of the healthcare team to ensure safe, timely, and quality care. Key responsibilities include physical assessments, nursing interventions, pain management, medication administration, specimen collection, nutritional support, and patient education on discharge needs. The nurse applies the nursing process based on evidence‑based best practices, adjusts care as needed, consults appropriately, and involves resource personnel when facing ethical or legal dilemmas.
Responsibilities- Deliver direct patient care in home health and hospice settings.
- Conduct physical assessments and develop individualized care plans.
- Administer medications, manage pain, and collect specimens.
- Provide nutritional support and teach patients about discharge planning.
- Apply evidence‑based nursing practices and adjust care as needed.
- Consult with interdisciplinary team and resource personnel for ethical, legal, or medical dilemmas.
- Perform comprehensive behavioral health evaluations, including mental status, mood, and behavior.
- Utilize a medical/psychosocial model to individualize care plans for patients with depression, anxiety, or dementia.
- Other duties as assigned.
- Licensed Registered Nurse in California.
- Current BLS certification.
- Valid driver’s license and a reliable, insured automobile in good working order.
- One year of home health experience preferred; two years of hospice or public health/home care experience preferred.
- Ability to work independently and collaborate with interdisciplinary teams.
