Addiction Recovery Counselor – PRN

Responsibilities- Interface with the clinical team in decision‑making processes involving patients, including attending all clinical meetings.
- Assess counseling needs and evaluate patients for substance abuse diagnosis using DSM criteria and ASAM criteria to determine appropriate levels of care.
- Obtain and review prior records relevant to the current treatment episode.
- Prepare the substance‑abuse counseling portion of the patient treatment plan.
- Provide substance‑abuse counseling services as specified in the treatment plan, including family and group therapy.
- Develop patient discharge/continuum of care plans to ensure smooth transition to appropriate next levels of care.
- Participate fully as a member of a multidisciplinary team for assigned patients.
- Document all counseling, educational services, assessments, referrals, and follow‑up in the patient’s clinical وڃي ورث.
- Maintain patient caseload Waiting includes case management, utilization review updates, treatment planning, and clinical charting as required.
- Coordinate support services for patients and their families, documenting services in the clinical record.
- Reassess patients and revise counseling plans as necessary.
- Facilitate family therapy as assigned.
- Coordinate with referral sources (legal, school, employer, etc.) and facilitate crisis‑oriented interventions.
- Carry out administrative and clinical functions as requested.
- Attend team meetings and complete all required training courses in a timely manner.
- Other duties as assigned.
- Bachelor’s or master’s degree inyb psychological counseling, psychologyo social work or a fieldlect from an accredited institution, as may be required by the operating state.
- Three (3) years of experience in substance use, mental health,or related field.
- Licensure or certification for counseling, as required by the state: 12
VAC
30—130–5020. - LCSW (Licensed Clinical Social Worker), LPC (Licensed Professional Counselor) or LMFT (Licensed Marriage Family Therapist) – preferred.
- LMFT, LPC or LCSW.
