Pay Rate:

Range $2120 to $2200 Weekly Gross pay based on a 40-hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments

Unit: Home Health

Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri, Weekends if and as needed

Position Requirements:

• Registered Nurse: 2 years
  - Required
• Recent/Current Home Health Nursing experience in a home health setting: 1 year within last 3 years
  - Required
• Home Care Home Base (HCHB) charting experience
  - Required (Can accept candidates with other EMR experience, provided they are willing obtain HCHB training offered through us prior to start)
• OASIS experience - R equired
• Must have valid Driver's License and Personal Auto Insurance
• Must be Covid Vaccinated orprovide medical or religious exemption

Education, License and Certification Requirements:

• ASN or an equivalent
  - Required
• BSN or an equivalent
  - Highly preferred
• RN License for State the state applying or Compact if permitted in that state
  - Required
• BLS Certification from AHA or American Red Cross
  - Required

Job Description:

• Provide and coordinate care for assigned Home Health patients in comfort of their homes.
• Develop and implement nursing care plans including Start of Care and admissions as assigned.
• Support patient recovery and ability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy.
• Empower patients and families to reach healthcare goals by educating them on disease management.
• Provide Home Health nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies,procedures, and protocols, as well as facility and corporate policies and procedures.
• Ensurephysicians' orders are legible if taken verbally and transcribed appropriately per policy and procedure.
• Ensure patients' status is assessed on an ongoing basis and pertinent information is gathered and documented.
• Notifyphysicians of significant changes in status.
• Document and complete charting for all patient visits within 24 hours.
