Job Summary

We are a friendly team looking for an enthusiastic Practice Nurse to join our team. You will undertake clinical practice using expert knowledge and clinical skills to deliver exceptional care to people accessing our primary care services. An NMC registration is mandatory.

Your Main Responsibilities May Include:
  • Long Term condition Management (e.g., asthma, diabetes, etc.)
  • Wound care
  • Cytology
  • Venepuncture
  • Hypertension Management
  • Routine immunisations
  • Involvement in QOF achievement
  • Infection prevention and control

As an autonomous practitioner, you are responsible for the care delivered, demonstrating critical thinking and clinical decision‑making in the management of patients. You will work collaboratively to meet the needs of patients, ensuring the highest standards of care and supporting the delivery of policy and procedures.

Newport Health Centre is based centrally on the Isle of Wight, operating across two sites:
Carisbrooke and the town centre practice, Dower House, Pyle Street. We support over 24,000 patients with a multi‑disciplinary team including GP Partners, Advanced Nurse Practitioners, Paramedics, MSK Practitioners, Mental Health Practitioners, Practice Nurses, HCAs, a Chronic Pain Coach, Care Coordinators, a Social Prescriber and many more roles. We are part of the Central and West Primary Care Network (PCN).

Job Responsibilities Leadership

Take responsibility for own development, learning and performance, participate in clinical supervision and act as a positive role model.

Promote and work to the Practices Vision and Values Clinical Practice
  • Assess, plan, develop, implement, provide and evaluate treatment programmes including:
    • Chronic disease management and, where appropriate, lead specialist primary care services.
    • Health promotion and well‑being plus individual plans for patients at risk of a long‑term condition.
    • Manage and treat long‑term conditions in line with national and local policies.
    • Prioritise health problems, intervening appropriately in complex, urgent or emergency situations.
    • Implement and participate in vaccination and immunisation programmes for adults and children, including patients travelling abroad.
    • Provide opportunistic wound care.
    • Diagnose and treat minor illnesses.
Delivering a Quality Service

Recognise and work within own competence and professional code of conduct. Produce accurate, contemporaneous records in line with legislation, policies and procedures. Prioritise, organise and manage workload in a manner that promotes quality. Deliver care according to NSF, NICE guidelines and evidence‑based care, assessing effectiveness through peer review, benchmarking and formal evaluation.

Communication and Relationships

Demonstrate sensitive communication styles, act as an advocate, produce written documents that evidence contribution to practice priorities, and manage confidential, complex information appropriately.

Analysis and Judgement

Ensure competence and training, inform managers of urgent matters, manage and assess risk, monitor work areas for safety and hazard compliance, ensure safe storage and disposal of vaccines and drugs, and apply infection‑control measures.

Planning and Organising

Prioritise own workload, embed effective time‑management strategies, and maintain flexibility for additional duties.

Service and Policy Development

Be aware of relevant practice policies and develop clinical standard operating procedures with leads.

Personal Development

Take personal responsibility for mandatory training and pursue additional training identified at annual appraisal.

Information System Use and Management

Use relevant IT systems (Systm One, Team Net, Word, Excel, Access) to support the role.

Freedom to Act and Autonomy

Work on own initiative, manage workload, adjust priorities and discuss matters in supervision sessions.
