Job summary
An opportunity has arisen for a Practice Nurse to join our experienced and friendly nursing team at our forward thinking and supportive practice. We have a dynamic team of nurses, healthcare assistants and phlebotomists providing chronic disease management and enhanced services to our 15,000 patient list. The ideal candidate will have proven experience in a primary care setting. We are looking for an experienced primary care Nurse.

To provide and maintain a high standard of nursing care for patients, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The duties will include but are not limited to day to day primary care nursing such as phlebotomy, ECGs, wound care, adult and child immunisations, B12 injections, BP monitoring as well as Enhanced Practice Nurse skills in chronic disease management, cervical screening and compression dressings.
This post reports to the Lead Nurse at the practice.
St Julians Medical Centre is a friendly, well-established GP surgery and training practice in Newport, caring for approximately 15,000 patients. We are looking to recruit an experienced, enthusiastic, flexible and reliable nurse to join our team. Our clinical team comprises: 6 GP Partners, 2 Salaried GPs, 2 GP Registrars, 3 Advanced Nurse Practitioners, An experienced nursing, healthcare and administrative team. We pride ourselves on having a supportive and collaborative working environment, with a strong focus on high-quality patient care, education and professional development.
As a training practice, we value the contribution that teaching and training brings to the whole team. We currently support two GP Registrars and have a strong culture of learning and development. We encourage our clinicians to develop their own areas of interest and are keen to support the successful applicant with their ongoing professional development. Salary - £37,000 to £40,000 pro-rata depending on experience Primary:- Able to document contemporaneous Consultations according to NMC guidelines using EMIS clinical system
- Maintain accurate clinical records in conjunction with legislation
- Ensures awareness of statutory and local clinical protection procedures, including systems of referral.
- Provide routine nursing care to patients as required in accordance with clinical based evidence
- Perform venepuncture according to local guidelines
- Request pathology services as necessary
- Process pathology results if required
- Provide a holistic approach to wound management and implement wound care in line with current evidence based guidelines and local wound protocol
- Suture removal
- Perform ECGs
- Ability to obtain and document informed consent (either verbal or written)
- Ensure infection control guidelines are maintained
- Confidentially of information gained at work must be always preserved
- Ensure read codes and templates are used effectively
- Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health
- Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
- Maintain chronic disease registers
- Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions
- Provide vaccination and travel medicine services
