- Complete required record‑keeping related to admission, transfer, and discharge of residents.
- Coordinate daily with nursing staff and support personnel to plan shift services, programs, and activities, ensuring residents’ care plans are maintained.
- Prepare and administer medications, reviewing medication cards for completeness and accuracy.
- Review resident care plans for appropriate goals, problems, and approaches, revising as needed based on nursing requirements.
- Plan, organize, and implement programs, goals, objectives, policies, and procedures necessary for quality care.
- Provide leadership to nursing personnel and conduct daily rounds of the unit to ensure staff complies with residents’ care plans and wishes.
- Develop and maintain rapport with nursing staff, other departments, residents, family members, and all individuals involved in resident care.
- Participate in facility surveys by authorized government agencies.
- Graduated from an accredited nursing program.
- Current, active, and unencumbered LPN license in Oregon.
- Knowledge of nursing and medical practices, laws, regulations, and guidelines relevant to nursing facilities.
- Accurate charting of nurse notes, reflecting care provided and resident responses.
- Experience with Electronic Medical Records and computer documentation systems.
- Maintain confidentiality of resident information in compliance with HIPAA.
