Licensed Practical Nurse (LPN) – Avamere Rehab of Newport

Avamere Rehab of Newport invites qualified Licensed Practical Nurses to join our team. The position is available as Full‑Time, Part‑Time, or PRN across various shifts. Located at 835 SW 11th St, Newport, OR 9736.

  • Complete required record‑keeping related to admission, transfer, and discharge of residents.
  • Coordinate daily with nursing staff and support personnel to plan shift services, programs, and activities, ensuring residents’ care plans are maintained.
  • Prepare and administer medications, reviewing medication cards for completeness and accuracy.
  • Review resident care plans for appropriate goals, problems, and approaches, revising as needed based on nursing requirements.
  • Plan, organize, and implement programs, goals, objectives, policies, and procedures necessary for quality care.
  • Provide leadership to nursing personnel and conduct daily rounds of the unit to ensure staff complies with residents’ care plans and wishes.
  • Develop and maintain rapport with nursing staff, other departments, residents, family members, and all individuals involved in resident care.
  • Participate in facility surveys by authorized government agencies.
Qualifications
  • Graduated from an accredited nursing program.
  • Current, active, and unencumbered LPN license in Oregon.
  • Knowledge of nursing and medical practices, laws, regulations, and guidelines relevant to nursing facilities.
  • Accurate charting of nurse notes, reflecting care provided and resident responses.
  • Experience with Electronic Medical Records and computer documentation systems.
  • Maintain confidentiality of resident information in compliance with HIPAA.
