Therapeutic Case Manager positions located in Newark, Miller's Run, Sutton, and Burke.
As the NKHS Therapeutic Case Manager you will be based out of the Cornerstone School day treatment program, integrating supportive services for students. This is an exciting opportunity to collaborate with a strong clinical team providing individual and family supportive interventions, and to develop treatment and transitional plans for students in Caledonia County for Cornerstone School and the Kingdom East Collaborative.
You will have the chance to be deeply connected in a small school setting while gaining valuable professional experience.
- Have or develop knowledge of local, state, and regional community resources.
- Meet with or have telephone contact with selected children and parents for evaluation, supportive interventions, crisis management, and communication of significant information.
- Coordinate clinical treatment team meetings.
- Develop and implement transition plans with referring agencies and/or school systems, and facilitate potential post-program placements.
