Payment
:
Standard on-call payment per shift plus additional callout allowance & mileage
Standard on-call payment: £100 per shift
Initial call out:
Additional £60 paymentAny further call outs:
Additional £20 per call out8pm cancellation: 4 hours pay will be due (£40)
Premiums will be payable in relation to the standard on-call payment (£100 element only) for Saturdays, Sundays and Evora Hospice recognized bank holidays
No payment for any shift cancelled due to no patientsor your non-attendance

Location:Community based role delivering care in patients’ homes throughout the catchment area (BT35 & greater Newry City)

Main Purpose of Job
Working as part of the Hospice Out of Hours Nurse Team, to be responsible for providing a nursing service to patients, who have a cancer diagnosis and are at end-of-life, as well as their families
To participate in the assessment, planning, implementing, and evaluating of care of patients in their own home encompassing the Hospice’s philosophy of palliative care
To collaboratively work with GPs, SHSCT District Nursing Team, Out of Hours, Marie Curie Service and SHSCT Specialist Palliative Care Team, to ensure effective communication in the provision of care for patients and their families at the end of life
Main Responsibilities
Clinical Responsibilities
Provide nursing care to meet patient’s needs and to enable symptoms to be well controlled at end of life
Maintain clinical competencies as defined by the needs of the service such as management of syringe drivers, administration of oral medication, administration of intravenous drug therapies, wound management, stoma care and basic life support
Work effectively with community professionals, hospital, and community palliative care teams, ensuring effective communication between all service providers
Build and maintain trusting relationships with patients, and their families/carers demonstrating empathy, dignity and understanding at all times
Ensure safe handling and administration of medicinal products in accordance with Hospice procedures, NMC requirements and RQIA standards
Provide advice and support for the carers and family according to their needs and the level of intervention required at end of life
Maintain excellent communication with patients, relatives and carers ensuring that the patient’s individual needs are met, reporting any issues or concerns, including safeguarding risks, to the Community Services Manager
Regularly work without supervision for all of your shift
Participate in the orientation, induction, and training of new staff and support, supervise and mentor staff
Refer any concerns around poor performance and practice to the Community Services Manager
Assist in maintaining own and others health, safety and security
General Responsibilities
Take responsibility for managing and prioritising own workload in conjunction with the Community Services Manager
Work independently, as part of the Hospice Out of Hours Nursing Team and also as part of the wider Hospice team
Promote and be an advocate for Evora Hospice at all times
Professional Responsibilities
Maintain professional registration with the NMC, adhering to the NMC code at all times
Ensure professional and personal development and accountability in accordance with the NMC Revalidation requirements, ensuring revalidation is completed 3 yearly
Practice in accordance with Evora Hospice policies and procedures
Actively participate in the annual performance review process to identify opportunities for learning and development
Participate in continual professional development as required, ensuring required training is kept up‑to‑date and renewed on a timely basis
Actively participate in clinical supervision and team meetings to ensure a contribution to the professional development of the nursing team
Maintain appropriate car insurance to cover business travel purposes and provide evidence of this at the time of annual renewal
