Clinical Lead​/Manager

The Frailty SDEC Clinical Lead is responsible for the operational and clinical leadership of the Frailty SDEC service, ensuring the delivery of safe, effective, person-centred care for older people with frailty. The postholder will provide visible clinical leadership, coordinate multidisciplinary working, support admission avoidance and timely discharge pathways, and ensure the service dleivers high quality care aligned with the Trust Integrated Cre System and National frailty standards

We require a motivated, compassionate and enthusiastic leader for our Frailty Same Day Emergency Care service. The service provides timely multidisciplinary assessment and intervention for people living with frailty, supporting admission avoidance, early supported discharge and safe transfer of care.

Proven leadership, excellent interpersonal skills and the ability to influence, support and motivate others are essential to this role.

• Provide a focused leadership role to manage staff and activities within Frailty SDEC.
• Act as a visible role model and maintain the highest standards of patient care.
• Lead and coordinate the nursing team within the service, demonstrating compassionate leadership and effective role modelling.
• Work collaboratively with the multidisciplinary team to deliver comprehensive assessment, treatment and discharge planning.
• Support admission avoidance and timely discharge using a Home First approach.

Frailty SDEC is a developing and dynamic service focused on providing timely assessment, treatment and care planning for people living with frailty who may otherwise require admission to hospital.

The team works closely with acute services, community services, therapists, social care, primary care, specialist teams and voluntary sector colleagues to deliver coordinated, person-centred care. The service supports patients and families to make informed decisions and aims to promote independence, reduce avoidable hospital admission and support safe care closer to home.

This is an exciting opportunity for an experienced clinical leader to help shape and develop Frailty SDEC, embed high-quality frailty practice and support the wider urgent and emergency care pathway.
