RN On Call

Good Shepherd Hospice is seeking a compassionate Registered Nurse (RN) for an On Call position serving patients and their families in Newton and surrounding communities. The RN provides nursing assessment, planning, and care to maximize the comfort and health of patients and families in accordance with the Interdisciplinary Plan of Care.

• Receive patient assignments from the Patient Care Manager or Case Manager. Communicate with attending physicians, hospice staff, and other agencies to coordinate care.
• Maintain communication with the patient, family, and healthcare team.
• Assess physical, psychological, social, and spiritual factors influencing patient and family health, and incorporate data into the Plan of Care.
• Keep accurate and up-to-date patient records, reflecting problems, plans, actions, and goals.
• Instruct patients and caregivers on medication administration, personal care, safety, and preparing for end-of-life events.
• Participate in team meetings, care plan meetings, and provide support during death.
• Provide 24/7 coverage for patients and families as part of the team.
• Evaluate hospice aides bi-weekly and LPNs monthly.
• Develop understanding of hospice philosophy and pain management through ongoing education.
• Perform other tasks as assigned.

• Current RN license in the state of practice.
• Hospice experience preferred.
• High integrity and confidentiality.
• Team player with adherence to care plans.
• Strong organizational, self-management, and critical thinking skills.
• Ability to pass background checks and hold a valid driver’s license.
