American Traveler is hiring an RN Float for Home Health with required Certified Home Care and Case Management experience and an active MA RN license.

• Home health setting providing care to patients ranging from newborns to over 100 years old
• Covers South Region including Allston, Belmont, Brighton, Brookline, Chestnut Hill, Newton Corner, Newtonville, Watertown, and West Newton
• 13-week assignment with scheduled shifts from 08:00-16:30
• Weekend rotation required approximately once every 6 weeks
• Must be willing to work holidays and on-call per department rotation needs
• Uses Epic for charting
• Uses UKG/Kronos for timekeeping

• Active MA RN license required (no pending or conditional licenses accepted)
• Minimum 1 year of Certified Home Care experience
• Minimum 1 year of Case Management experience
• Current AHA certifications required
• Reliable, independent transportation required for home visits
• Primary source verification of license required within 30 days of start, updated upon expiration (NURSYS and BON required)

• Provide clinical home visit services including patient assessment, care plan implementation, and ongoing patient/family education and support
• Perform admissions and case management duties per MGB Healthcare at Home policies, including timely electronic documentation and interdisciplinary communication
• Flu vaccination required with no religious or medical exemptions allowed
• Local candidates accepted if residing within 60 miles of Massachusetts General Hospital in Boston
• No validated parking available at Boston locations; public transportation or paid parking required
