American Traveler is hiring a Home Health RN Float for a 13-week assignment requiring 1 year of certified home health care experience and an active MA RN license.

• Home health setting covering North Region Scheduling area including Lynn, Nahant, and Saugus
• Float position requiring travel to assigned patient cases
• Day shift schedule from 08:00 to 16:30
• Weekend rotation required once every 6 weeks
• Uses Epic for charting

• Active MA RN license required, no pending or conditional licenses accepted
• AHA-issued certifications only
• Minimum 1 year of experience in a certified home health agency required (hospice, private care, or standalone case management experience does not qualify)
• Minimum 1 year of case management experience required
• Ability to travel to assigned patient cases

• Assess patient conditions, implement care plans, and provide ongoing education and support to patients and families
• Perform admission processes and case management duties in accordance with home care policies and standards of practice
• Maintain thorough and timely electronic documentation and interdisciplinary communication
• Local candidates considered, with permanent residence within 60 miles of Boston required for local rate eligibility
• Flu vaccination required with no religious or medical exemptions allowed
