American Traveler is hiring a Home Health RN Float with certified home care and case management experience to serve patients across the North Region.

• Home health float position covering Cape Ann, Ipswich, and North Pod service areas including towns such as Gloucester, Ipswich, Haverhill, and Newburyport
• 13-week assignment with scheduled shifts from 08:00-16:30
• Weekend rotation required once every 6 weeks
• Ability to travel to patient homes across assigned coverage areas required
• Uses Epic for electronic charting and documentation

• Active MA RN license required, no conditional or pending licenses accepted
• Minimum 1 year of experience in a certified home health agency specifically required
• Minimum 1 year of case management experience required
• Hospice, private care, or standalone case management experience does not meet qualification requirements
• AHA certifications only accepted
• Flu vaccine required with no religious or medical exemptions allowed

• Responsibilities include assessing patient conditions, implementing care plans, and providing education and support to patients and families
• Perform admission processes and case management duties in accordance with home care policies and standards of practice
• Deliver patient care consistent with organizational philosophy and community health practice standards
• Requires thorough and timely electronic documentation and interdisciplinary communication
• Local candidates accepted if residing within 60 miles of Boston, MA, with applicable rate adjustment
• Uses UKG/Kronos for timekeeping
