Job Summary
:
The Care Team Administrative Assistant provides support to the Care Teams specifically as it relates to acting as the typist for care planning meetings; preparing and providing documents for the team to reference and utilize for and after care planning meetings; and administrative support in other aspects as directed. Position reports directly to the Center Manager.

Education
:
High School Diploma required.
Experience
:
At least 1 year’s experience working with the frail elderly population. Experience working in healthcare and a team environment preferred.
Key Skills
:
Must be proficient in typing, transcribing, and basic Microsoft applications (specifically, Word, Teams, Excel, & Outlook). Must possess excellent skills in communication (written and verbal). Must be knowledgeable in medical terminology.

Immunizations
:
Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
Key Responsibilities
:
- Attends weekly care planning sessions with each care team.
- Prepares and provides care plan facilitator’s work sheets to facilitator prior to scheduled care planning sessions.
- Types/transcribes participant care plan plans during weekly team meetings.
- Prints/distributes care plans to team members for review with participants/caregivers.
- Assists center managers with administrative tasks regarding care plan implementation and evaluation.
- Communicates with team members regarding administrative tasks as needed.
- Assists with the maintenance of participant medical records as needed.
- Assists with updating participant demographic information in the electronic medical record system.
- Supports Senior TLC’s mission to encourage and support the quality of life of seniors wishing to continue living in the community; its vision to be the preferred provider of individualized care for seniors in the community; and its values of respect, integrity, accountability, compatible goals, and compassionate care.
- Other duties as assigned.
