Position Overview
St. Mary Home Care (a member of Trinity Health at Home) is seeking a Part‑time Occupational Therapist to provide skilled services to homebound patients in the greater Bucks County area. The role requires working 3 days per week, 24 hours total.
Compensation and Benefits- Pro‑rated $2,500K Sign‑On Bonus for part‑time employment (not applicable to internal or recent former employees or employees sourced from outside firms).

- Provide one‑to‑one occupational therapy care in patients’ homes.
- Collaborate with interdisciplinary team members to ensure patient needs are met and quality of care is achieved.
- Act as patient care manager overseeing primary care delivery to a select group of patients.
- Develop, plan, and administer physician‑prescribed occupational therapy services to restore function, relieve pain, and prevent disability following illness, disease, injury, or surgery.

- Graduated from an approved occupational therapy program;
Bachelor or Master’s preferred. - Current licensure as an Occupational Therapist in the state of Pennsylvania.
- Minimum one year of professional clinical care experience; recent home‑care experience preferred.
- Current driver’s license and reliable transportation.
- CPR certification required.
